RESERVATIONS NEEDED BY MONDAY, DECEMBER 7, 2009

1) Check out the ticket options above and determine the number of tickets and ticket level you are interrested in.

2) Email Renee@RACI.org the following information:

• # of tickets you will be buying
• Ticket leve
• Full name
• Company name
• Phone number
• Name of all guests

3) You can mail payments to RACI at 1620 E Hoffer Street, Kokomo, IN 46902. You can drop off payments at the RACI office in Kokomo at 1620 E Hoffer Street or the RACI office in Marion at 1125 N Western Avenue.

• Make checks payable to RACI
• Subject: RACI Christmas party
• Full payment is required before the start of the event

4) Pick-up your reservation packet at the door upon arrival (at the Kokomo Country Club)
