The existing office is located at: 3733 University Blvd., Suite 100, Jacksonville, FL 32217. It will close at noon on Friday, Dec. 12, 2008.

The office will reopen on Monday, Dec. 22, 2008, noon, at the new location: 7185 Bonneval Road., Ste. 1, Jacksonville, FL 32256. The office hours will be M-F from 9-4.

During the transition, the office at 1685 Dunn Avenue, Jacksonville, FL 32218, will be open to provide services.
